What Is the Urinary Tract?

The urinary tract consists of the kidneys, ureters, bladder, and urethra. The kidneys filter blood and excrete waste as urine; the ureters carry urine to the bladder; the bladder sits in the pelvis (in men, just above the prostate); and the urethra carries urine out of the body shorter in women, and passing through the prostate and penis in men. UTIs can occur in any part of this system and are most often caused by bacteria, though fungi and viruses can also be responsible. UTIs are the second most common infection type in humans, and can involve the urethra (urethritis), bladder (cystitis), or kidneys (pyelonephritis potentially life-threatening).

Risk Factors

Obstructions (enlarged prostate, kidney stones, cancer), gender (women are more at risk due to a shorter urethra, while UTIs in men are less common but more serious), sexual activity, bathroom hygiene, spermicides, condoms (friction/irritation, though still important for STI prevention), diaphragms, diabetes, loss of estrogen after menopause, and prolonged catheter use.

Symptoms

Lower UTI (urethra/bladder): burning during urination, frequent urination with scant amounts, bloody or cloudy urine, strong odour, pelvic pain in women, and rectal pain in men.

Upper UTI (kidneys potentially life-threatening, with sepsis risk): upper back or side pain and tenderness, chills, fever, nausea, and vomiting. Pregnant women experiencing UTI symptoms should see a doctor immediately, given the risk of premature delivery, high blood pressure, and infection spreading to the kidneys.

Diagnosis

Diagnosis typically involves a “clean catch” urine specimen, examined for white blood cells and cultured for bacteria. Upper UTIs may require a CBC or blood cultures, while patients with recurrent UTIs may need an ultrasound, IVP, or cystoscopy, with possible biopsy to rule out bladder cancer.

Treatment

Treatment is antibiotic-based oral antibiotics for lower UTIs, IV antibiotics for upper UTIs. Urine cultures guide antibiotic choice if resistance is suspected.

Prevention

Wipe front to back, drink 6-8 glasses of water daily, drink water after sex, avoid holding urine for long periods, maintain daily hygiene, prefer showers over baths, and wear breathable underwear.
