What Is Hematuria?
Hematuria is the presence of red blood cells in the urine. It can be visible (“gross” hematuria) or non-visible (“microscopic” hematuria, seen only under a microscope). Microscopic hematuria is often an incidental finding on routine tests, while gross hematuria usually prompts a doctor visit directly. Blood can originate anywhere in the urinary tract kidneys, ureters, bladder, prostate, or urethra and occurs in an estimated 2.5-21% of the population. Often no specific cause is found, but it can be a marker for infection, stone disease, or urinary tract cancer. Risk factors include smoking, radiation, overuse of certain pain medicines, and certain chemical exposures.
Common Causes
Bladder cancer, kidney cancer, prostate cancer, ureteral cancer, urethral cancer, urinary stone disease, UTI, pyelonephritis (kidney infection), BPH (enlarged prostate), renal disease, radiation- or chemical-induced cystitis, urinary tract injury, prostatitis, and exercise-induced hematuria.
Diagnosis
Visible hematuria usually prompts immediate medical attention, while microscopic hematuria is detected via a urine dipstick test and further evaluated if 3 or more red blood cells per high-power field are found on 2 of 3 specimens. Further tests may include a careful history and physical exam, urinalysis/sediment exam, urine culture (if white blood cells are present), urinary cytology, and a blood test (serum creatinine) to check kidney function. A full urologic evaluation includes ultrasound or CT imaging of the kidneys and ureters CT is preferred for kidney masses and stones, and CT urography allows the kidneys and ureters to be examined in one test. MRI or retrograde pyelography may be used if creatinine is elevated or a dye allergy is present. Cystoscopy is required to evaluate the bladder, since imaging alone cannot assess it this is usually done in-office under local anaesthesia with a flexible cystoscope.
What Happens If No Abnormality Is Found?
No cause is found in at least 8-10% of cases (some studies show a higher rate). A urologic malignancy is later discovered in 1-3% of patients with an initially negative work-up, so follow-up is recommended: repeat urinalysis and cytology at 6, 12, 24, and 36 months, with immediate re-evaluation (cystoscopy or imaging) if gross hematuria, abnormal cytology, or irritating symptoms recur. If no symptoms appear within three years, no further testing is needed.
