What Is a Urethral Stricture?
A urethral stricture is a narrowing of the urethra the tube that carries urine from the bladder through the penis.
Causes
Urethral strictures are usually caused by tissue inflammation or scar tissue. Risk is higher in boys who had hypospadias surgery and in men with penile implants. Straddle injuries such as falling on a bicycle bar or a direct impact to the groin are a common cause. Other causes include pelvic fractures, traumatic catheter insertion, radiation, cystoscopy or prostate surgery, and sexually transmitted infections such as gonorrhoea and chlamydia.
Symptoms
Weak urine flow or reduced volume, sudden and frequent urges to urinate, a sensation of incomplete emptying, a frequent start-stop stream, pain or burning during urination, incontinence, pelvic or lower abdominal pain, urethral discharge, penile swelling or pain, blood in the semen or urine, darkened urine, and in severe cases a complete inability to urinate, which is a medical emergency requiring immediate attention.
Diagnosis
Diagnosis is made using a flow test and flexible cystoscopy, or a urethrogram a special X-ray using contrast dye.
Treatment
Treatment options include widening the stricture with special dilators, or cutting the stricture with lasers. For longer or more severe strictures, surgery (open urethroplasty) may be required this removes the affected tissue and reconstructs the urethra. Results vary depending on the size of the stricture.
